Estrella XXVIII A&S Competition Pre-registration Form

Instructions…

Pre-registration for the Estrella War XXVII Arts & Sciences Competition is required. Completed pre-registration forms must be received by Saturday 3 March 2012.

PLEASE NOTE: This form is designed for electronic submission. The information you enter here will be e-mailed to the competition steward. Please follow these instructions for using this e-form:

Read the form carefully and enter the information needed into the fields provided (or by selecting appropriate buttons or checkboxes). Note that required fields are indicated by a red asterisk (*)—these fields must be completed in order to successfully submit the form. Note that personal contact information requested will remain confidential—this is only needed to contact the entrants in case more information about an entry is needed or an entry is mistakenly left at the war site.
Please double-check the information entered and choices made to be sure that you have not made errors.
Press the <SUBMIT> button (at the very bottom of the form) to dispatch your pre-registration. If this form (refer to form title above), with your information still there, does not disappear, you have not given correct information for one of the required fields. Errors (in red) at the top of the form and under each incorrect field will be shown—you will need to complete the form and hit <SUBMIT> again. Correct submission will return you to the previous "Estrella War A&S Competition" page.

If a valid return e-mail address is included, a manual confirmation of receipt will be sent by the competition steward to the e-mail address provided when the pre-registration e-mail is received/read. Entrants are assumed responsible for checking to make sure their pre-registration information has been received.
